Would anybody be wiling to share your companies Personnel Manual or know where I could get a good Personnel Manual template to build mine from?
I'm also interested knowing what type of incentive programs you have for your Managers.

I use Bob Copper's material for the operations manual. It is very thorough. I haven't looked at his employee handbook b/c I use one that was created as a part of my ADP HR services - but I'm sure it's equally good or better. When you purchase any of the printed material, you get a digital copy as well that you can modify as you see fit.
